How to complete a IRS Change of Address
- Check all boxes that apply to the change in your home mailing address, including for individual income tax returns or gift estate returns
- If you changed your name due to marriage or divorce, make sure to complete line 5
- Ensure you provide complete address information, including any apartment or suite number
- If your spouse's address is different, enter their information as well
